Syarat dan Ketentuan Umum

1. Participants are required to:

• Like Facebook account Ciptadent
• Follow Twitter account @CiptadentID

2. In providing support, Rumah Kuat Ciptadent activity participants are open to anyone as support to help #CiptadentKuatkanIndonesia
3. In the Rumah Kuat Ciptadent Weekly Quiz series, participants are not employees and families of PT. Lion Wings Indonesia, the agency, and all parties involved in this activity
4. By participating in this activity, participants are willing to guarantee all information submitted is true and complete
5. The information submitted on the form does not contain ethnicity, religion, race, and inter-group relations, which are defamatory, degrading, threatening, and violate applicable norms.
6. PT. Lion Wings Indonesia as the organizer has the right to conduct a feasibility test of all participants and disqualify participants if it has a reason that the participant has violated one of the provisions.

Mechanism:

1. Visit the Ciptadent website: www.ciptadent.co.id. Then click the banner "Rumah Kuat Ciptadent".
2.  Fill in your personal data in the available form and make sure the personal data provided is correct.
3. Then click “Moms' Support.”
4. You will get a certificate of appreciation that can be shared on your social media on Instagram or Facebook
5. Share the certificate of appreciation for the support and mention @ciptadent with the hashtag #RumahKuatCiptadent #KuatBersamaCiptadent.
6. Then click "download" to get a voucher so that it can be used to consult a dentist from Halodoc.
7. After getting the voucher, you can directly scroll down or access the Halodoc application to consult a dentist.

How to Redeem Voucher:

1. Scan the QR Code on the voucher
2. Download the Halodoc app if you don't already have it
3. Choose a chat with a doctor
4. Choose an available Dentist
5. Enter promo code HALOCIPTADENT payment page
6. Finish the transaction and start a consultation with the doctor

Gifts:

1. The gift obtained from clicking "moms' support" is a voucher to consult a dentist from Halodoc.
2. Vouchers can be obtained directly after filling in personal data on the form.
3. Vouchers cannot be cashed or redeemed and transferred.
